3. Medical questionnaire

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

There are 2 ways to sign up for Online services.....

1. You don't visit the surgery and use the NHS app.

You can sign up for an NHS login and use you smartphone or tablet to send a photo of your photo id and record a short video clip. This allows you to access online services through the NHS app. Click on the link below for more information.


2. You visit the surgery and show a picture ID to our receptionist.

Visit reception with a photo ID such as driving licence or passport and we will print you an access key to use in an access app or on your computer. Please visit from 12:30 to 14:30 if possible as the front desk is quieter. Click on the link below for further details

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice. Please bring a photo ID so we can get you signed up for online access when you attend.
